Midland Dental - John Carter DDS - Financial & Insurance Information 

We appreciate your selection of our office to serve your dental health needs, and are dedicated to the idea that dentistry can be a pleasant experience! We are also committed to providing you with the highest quality dental care possible with precision, excellence, and caring.

In order for us to provide that care, we will be happy to help you maximize your insurance benefits by submitting your claims for you. While filing of your insurance claims is a courtesy to our patients, we want you to know that insurance companies make no guarantees of payment for services.  They provide us with an outline of "estimated benefits", which allows us to give our patients an approximate amount of your personal treatment costs.  Any portion that is not covered by your insurance company will remain the responsibility of the patient/parent. We also require that if your insurance company has not paid the claim •within 90 days, the balance will then become the patient's responsibility, and you will need to contact your insurance company. We will continue to aid you with any necessary documentation that your insurance company is requesting.

Estimated patient portion is due at the time of service. For your convenience, we accept most credit cards, as well as providing an outside financing company (Care Credit/Capital One).  Care Credit/Capital One are dental credit cards that are interest free for up to 3 months, however, they also  have  installment  plans  that    go  up  to 18 months. This allows our patients to have a comfortable monthly installment and receive their necessary dental treatment.

A service charge of 1.5% per month will be added to any account that has an outstanding balance of 60 days or older.  Please be advised that you will be responsible for any collection charges, if the need would arise to place your account with a collection agency. Returned checks will be subject to a $20.00 fee.  We do understand that mistakes can occur, and request that you contact the office to avoid any misunderstandings with regards to your account.

We respectfully ask our patients to be prompt and keep their scheduled appointments. It is difficult to fill appointments without the proper notification of 48 hours. (2 business days)  If a patient continues to miss scheduled appointments, we will be forced to dismiss them from our practice. We reserve the right to charge a fee of$45.00 for missed or late canceled appointments.
